If you would like your Medicare D Prescription Drug Plan, Supplemental Plan or your Medicare Advantage Plans reviewed please contact Rita Kowalchik at (203) 630-4222 or Julie Johnson at (203) 4244.

This office provides social work services to Meriden residents. Services include:

* Developmental screening for children three and four years old
* Geriatric assessment
* Information and referral
* Health insurance counseling
* Short term case management
* Community education on medical access issues
* Screening and assistance applying for entitlement programs
* Assistance with employability issues
